The design of the match is for nobody to enter and no one to leave, but that rarely happens in actuality.

There are no disqualifications in the match itself and the only way to win is by pinfall or submission. Every match has had a winner and a loser with the exception of a Raw in 1998, and then two pay-per-view matches.
